The DM & HO's and the Officers mentioned in the address entry are informed that medical bills are forwarded to the DPH & FW TS Hyderabad for sanction some instances bills are received without necessary documents due to which the sanction orders Medical reimbursement Claim . In could not be accorded and the bills are returned for want of proper documents .
In view of the above it is requested to forward the Medical bills for reimbursement claim with the following documents / enclosures in duplicate
1.Appendix - II with complete residential address duly signed by concerned D.D.O.
2.Emergency Certificate
3.Essentiality certificate
4.Dicharge Summary
5.Detailed Medical Bill
6.Non Drawl certificate signed by D.D.O. Concerned
7.Dependent Declaration of the employee ( In case of dependent medical reimbursement )
8.Hospital Recognisation Proceedings
9.Pension Payment Order in case of retired employee
10.Entire Medical Bills Claim in duplicate
It is informed that non submission of any of the above documents / enclosures the Medical bills will be returned . Further the bills should reach this office for reimbursement claim within ( 6 ) months from the date of discharge . // Attested //
